Your life will depend on your hunger, fatigue, body temperature, etc. The cold weather, warmth of your body and the freezing water will be your enemies. Oh, there is no fast travel and health regeneration! You need to sleep to level up, you will carry less weight, creatures will carry threatening new diseases and previous diseases are now even more debilitating. Shrines will no longer cure diseases for free! Come on guys, how hard can it be?
Watch the skies, traveler.
